This happened about 5 years ago when i was working for Baxter Auto Parts full time. We had just received a new NOS bottle (Yes, an actual Nitrous Oxide Systems bottle) with a safety valve attached and were filling it for a customer. I was filling it at the NOS fill station. I hooked up the bottle and the large bottle was pressure filling the small bottle. I looked at the gauge and the bottle pressure was at 600psi. I went to hook up the air hose to the nitrous pump and just as I reached over the bottle to plug in the air hose, there was a loud "POP" and a cloud of white smoke. The safety valve failed at 600psi and since the safety valve had the cap on it, the spray didn't shoot directly outward, it sprayed in 4 direction (cap has 4 small holes in it). It sprayed me on my arm which was bare, and my side. My side wasn't that bad, but my arm was in the spray for a split second. Here is a pic the day it happened.
